Established patients should:
Make sure your phone number and address are correct or we may not be able to get in touch with you about important medical information.
If possible, register and check in through your phone prior to your scheduled appointment using Phreesia, to expedite your wait time.
If you have insurance, please provide all insurance cards at each appointment and let us know if there have been changes.
If you do not have insurance, please update your income information every six months if you are on the sliding scale. Documentation that will need to be provided can be found here.
